PROCEDURE and INFORMATION FOR ENROLLMENT
Enrollment for the 1st Semester, AY 2011-2012:

Information on Tuition and Other Fees:
- Tuition Fee: PhP 1,000.00/unit
- Miscellaneous: PhP 995.00
- Student Fund: PhP 46.50
- Deposit: PhP 100 (new students)
- Entrance: PhP 30.00 (new students)
Steps Prior to Enrollment:
For Continuing Students:
- Get clearance form from the IM Office and secure clearance from IM, Library, OSFA, OUR and Chancellor's Office. Get copy of your last semester's grades from Ms. Carmen Ramos.
- If returning from Leave of Absence (LOA), get Admission Slip from the Registrar if with conditions. If you are of good standing, from Ms. Carmen Ramos.
For New Students:
- Get Certificate of Admission and Student Directory Form from Ms. Carmen Ramos.
- Proceed to the CSS Admission Table and get your Student Number and then return to Ms. Carmen Ramos for your Admission Slip.
Enrollment Procedure:
- Admission: (Galleria, Auditorium or Lobby, in case it rains)
Get your Admission Slip and Preliminary Registration Form (From 5a) from Ms. Ramos. For old students, show your clearance and copy of grades. For new students present your eligibility for admission.
- Advising: (Institute of Management Conference Room)
For old and new students see IM professors for advising.
- Enlistment: ( Institute of Management Front Desk Counter)
See Ms. Carmen Ramos for enlistment. Have your Form 5a initialed per subject.
- Issuance of Form 5: (Auditorium)
Proceed to the Auditorium, secure your Form 5 and accomplish it properly. Use only black or blue ink, and make sure that your writing impresses on the carbon form. Make sure to supply all required information. Go back to your adviser for signing.
- Checking of Form 5: (Auditorium)
Proceed to Checking Table. Present your Form 5 and Form 5a.
- Assessment: (Auditorium)
Proceed to the Auditorium for Assessment (Assessment Table) and look for Ms. Pen Facunla.
- Payment: (Cashier's Office)
After assessment, proceed to the Cashier's Office for payment. Indicate Official Receipt number in appropriate boxes in your Form 5.
- Issuing of Class Cards: (Auditorium)
Get class cards and fill up properly.
- Stamping: (Auditorium)
Present your Form 5, Official Receipts and Class Cards for stamping.
- Countersigning of Student ID and Library Card: (Library)
For new students, apply for your Student ID at the Registrar's Office at Room 203 (below the auditorium). For old students, go to the Library for countersigning of ID (show Form 5).
